This is the traditional Case Management model. Availability of this service is limited and requires meeting eligibility criteria. This will be explained during the intake process. You will work one on one with your case manager and create an Individualized Service Plan which will guide treatment.

At Catholic Charities we offer three different types of case management services our intake clinicians will meet with you where is most comfortable for you.
Once you call us for an appointment or we receive a referral on your behalf you will be contacted by our intake team to schedule an intake appointment. At your intake appointment you can expect to be asked questions about your background, mental illness and current struggles. Due to the amount of information that must be gathered during this appointment, you can expect this meeting to last 1-2 hours. After intake is completed you will be assigned a case manager within 7 business days.
