Following an initial assessment to determine your child’s medical, mental health, educational, social and recreational needs, your case manager will develop an individualized treatment plan, focusing specifically on the goals and needs identified by you and your child.
With a better understanding of your situation, strengths and needs, your case manager can then help you find services and support, make referrals, assist with team meetings and special education needs, and generally monitor your child’s progress to ensure the effectiveness of the plan in meeting your goals. Right from the start, you are involved and up-to-date in your child’s progress every step of the way.
Our case management teams are made up of credentialed case managers working under the supervision of a licensed social worker/counselor.
